ECHOCARDIOGRAMS: AVAILABLE ON-SITE FOR ALL ATHLETES. THERE WILL BE AN ADDITIONAL $50 CHARGE.
ImPACT BASELINE TESTING: Due to time constraints, we will NOT offer ImPACT Baseline testing on May 20. IF you would like to have a Baseline done, please contact Jonathan Utsey at [email protected] to schedule an appointment.
All students must have a complete Certificate for Athletics on file in the Friendswood High School Athletic Training Room before they are allowed to participate in any tryout, practice session (including Athletic Periods), scrimmage, game or match.
To save time, please complete all sections of the forms except for the actual Physical Examination section prior to May 20.
Please see your personal physician, if you are unable to make this date and time.
Physicals done prior to May 20 WILL NOT be accepted. This physical is for participation in the 2017-2018 school year.
